In this episode I am joined by Jules and Annie from the Acute Care at Home team (@_VWNorth). They talk us through their virtual ward criteria, how they help people remain at home or to be discharged from hospital sooner. They talk through some of the challenges and also the amazing feedback showing how vital their service is in preventing unnecessary hospital admission but providing top quality care in a patients own home.

In this episode we are joined by Head of Nursing at County Hospital, Rebecca Ferneyhough as she talks us through the exciting application of the Johns Hopkins Activity and Mobility Programme. Inspired by a piece from the The Book About Getting Older by Dr Lucy Pollock, Dr Alice Holt, Dr Sarah Kelt, Dr Eleanor Cross and Ian Tyrrell explore how to approach having difficult conversations in regards to Respect documents and resuscitation. They explore how prepared they felt when first approaching these conversations and how they become easier after time.
