This week, Stan and Michael reflect on the growing public health challenge of meeting the needs of older people with diabetes globally. Possible deintensification of treatment and the risks of hypoglycaemia may be two important things to remember. We also touch on the relationship between impaired awareness of hypoglycaemia and ‘diabetes distress’.

We were blessed to be joined in studio this week by another significant South African, ‘superhero’ Karyn Maughan, specialist legal journalist and analyst at News24, well-known for her analysis of high-profile and far-reaching legal matters. Karyn is also a documentary producer, broadcaster, author of three books including, "Nuclear: Inside South Africa's Secret Deal", and she lives with type 1 diabetes. Karyn shares

In this bumper Episode we have two guests:
First up is world record-breaking ultramarathoner and Comrades Marathon legend, archaeologist, political activist, volunteer, author, sports columnist motivational speaker, and the chief executive officer of the South African Sports Trust, Bruce Fordyce – although very proud of his Comrades achievements, parkrun is Bruce’s ‘driving force’ and favorite topic. We discuss the potential of parkrun to change the lives of people through the physical activities of running and walking and of volunteering, all well supported in the medical and diabetes literature. Bruce makes a powerful case for the frailer, older person with diabetes feeling most welcome at parkrun, encouraging them to be physically active in an inclusive, safe and motivating environment.
Our second guest is recently appointed CDE Academy Administrator, Elré Clarence. Elré shares deep insights into her life with diabetes since the age of 10 months. In an ‘experiment’ to role model a developmental approach to the management of diabetes, Elré graciously relates aspects of her life with diabetes with reference to Erik Erikson’s Stages of Psychosocial Development

This week Stan shares some insights from a medical education conference he attended in Cardiff, UK - ‘Learning together for Clinical Excellence’. These insights will inform the programme for the 25th CDE Postgraduate Forum in Diabetes Management to be held from 27 - 29 October 2023 at the Maslow Hotel in Sandton. Stan also shares an emerging concept of ‘One Health’, which helps us to understand the profound health interactions between man, the environment and animals.

This week Stan and Michael discuss the potential uses of GLP-1 receptor agonists like semaglutide in fields beyond diabetes, like psychiatry, and wider roles for the long-used molecule, metformin. They also offer a high-level review of, and insights into the biggest medical barrier to the successful management of diabetes, and potential acute complications of treated diabetes, hypoglycaemia and possible hypoglycaemia unawareness.
We meet our fascinating studio guest, Robby Richardson, serial adventurer and now CEO at Houghton Golf Club, Executive Committee Member of the Club Management Association of Southern Africa (CMASA) and living with type 1 diabetes for 46 years.
Robby relates
His diagnosis of type 1 diabetes
His early experiences with porcine (pig) insulin, and urine testing for glucose which offered very little insight into his glycaemia or understanding of his diabetes
Living with diabetes for extended periods in many countries - United Kingdom, Norway, Belgium, Morocco, Portugal and eventually South Africa
Working long hours as a disc jockey in nightclubs and later managing bars
Experiences of severe hypoglycaemia and some serious accidents and injuries because of hypoglycaemia unawareness – the effect of this on work life, especially in a leadership position
How his dog helped rescue him during a severe episode of hypoglycaemia
How an insulin pump, a team approach to diabetes, and diabetes education has changed his self-management and understanding of diabetes
How, unlike many others, alcohol has never played a part in his episodes of hypoglycaemia
That a full and ‘normal’ life can be lived by finding the right people and resources to help you live with diabetes
We also discuss how presumed hypoglycaemic coma should be managed:
Given the potential cardiovascular disease risk, especially in people with type 2 diabetes,
the first question to ask is, “is the person still breathing?” If not, commence cardiopulmonary resuscitation and call an ambulance
If they are, turn the person on the side to maintain an open airway, check for low blood glucose and treat accordingly.
Importantly, for people in the community, insulin should never be given – always glucose or glucagon injection

This week Stan and Michael remind our listeners on the bidirectional relationship between oral health and diabetes and periodontal and cardiovascular diseases. Also about vaccination for human papilloma virus (HPV) to change the future for young people by preventing a common sexually transmitted infection, which may also raise long-term risk for certain cancers.
Stan then interviews Michael on some of the ‘Core Concepts’ he has identified in diabetes care over the last nearly 30 years:
“Diabetes is a Common, Costly and Challenging Chronic health Condition. Most cases of diabetes are Classified into 4 major aetiological types: Type 1, Type 2, Gestational Diabetes (GDM) and ‘Other’. Although people with undiagnosed diabetes or whose diabetes is not well-managed may complain of Classic symptoms, most do not - they do not know they have the condition, and they continue to feel relatively well despite underlying health risks.
Undiagnosed or uncontrolled diabetes can result in an increased risk of acute metabolic and chronic Cardiovascular and Cancer Complications. Further bad news is that we don’t have any satisfactory Cure at present. But the good news is that once diagnosed, diabetes is Controllable - thus potential complications may be prevented or delayed. Control can and should be achieved and maintained in the Community, using 3 Cornerstones of treatment together with regular Checking of outcomes.
But that’s not all. Choice Care should be a ‘Team’ facilitated process that has Continuity, is Congruent and which strives for Concordance between the Team and the person with diabetes. Good Communication is vital, and this will give that person the best chance of achieving Confident Self-Care. However, many strange and mythical Conceptions are associated with the condition. Before successful diabetes management can be achieved, Change in our and our client’s ways of thinking is required, through a Continuous process of Conscious and Contemplative self-review.

In a landmark Episode, we meet a grand older lady of immense fortitude and wisdom, Elizabeth Lane. Now 86 years old and having lived with type 1 diabetes for 66 years, Elizabeth speaks with authority on past years living with this chronic condition, both without and with the benefits of modern treatments and technologies, and the wonderful evolutions and revolutions in insulin therapy and sensing technologies over the decades.

How she and her father lived for many years without the benefit of being able to measure blood glucose
Her experience of the first blood glucose meter… vast, heavy, blue and cumbersome, but a ‘game-changer’

The ‘magic’ of her new hybrid closed loop insulin pump which has made her life ‘so much easier’, and which drastically decreased her experience of hypoglycaemia
Her ability to bear children, live a long and healthy life and work in a professional role
Her frustration with the well meaning but misguided ‘guidance’ from others
The importance of people ‘unlearning’ their misconceptions
The importance of consistency and routine in diabetes self-care

They also discuss a recent news article highlighting the lack of foundational functional literary in young South African schoolkids, compounded by parents and caregivers abdicating their education role by not reading and storytelling. Such ‘socio-economic determinants of health’ may have serious implications for the future of these children in society and in the workplace, and in the self-management of their health. Practically, we see evidence of this problem among health professionals – as a result, not all health professionals within the same profession are equal in their functional literacy, and in their resultant clinical insights and abilities.
