This was a fun, but technical episode.    Cyrus Khambatta has a PhD in nutritional biochemistry from UC Berkeley. Robby Barbaro has a master’s degree in public health. Both have type 1 diabetes and have been featured in plant-based documentaries like Forks Over Knives.    Though Robby and Cyrus have been able to improve their blood sugar control with a fruit heavy plant-based diet, I had some concerns about their approach (inadequate protein, nutrient deficiencies, etc..) so I invited them on the show. Even with almost two hours we didn’t have time to get into everything but we did talk about the biochemical basis of insulin resistance vs glucose sparing in detail and addressed the false notion that low carbohydrate diets cause pathological insulin resistance head on.    I hope you all enjoy this episode and I look forward to doing more animal vs plant “debates” with plant based advocates in the future. I don’t want my podcast to be an echo-chamber!  Time Stamps: 8:54 Start of the Podcast  10:01 Cyrus and Robby's story. 16:11 Have either done a low carb ketogenic style diet ? 28:16 Glucose and mean amplitude glucose excursions.  33:26 Paleolithic ketogenic diet and type 1 diabetes(a case study). 39:50 Is there long term studies on the carnivore diet? 44:10 Insulin resistance  54:00 Insulin resistance and hyperinsulinemia  59:31 Are low carbohydrate diets effective in combatting insulin resistance? 1:06:47 LDL cholesterol and cardiovascular disease. 1:18:20 Can blood glucose sensitivity improve on a high carb plant based diet? 1:19:46 Blue Zones  1:27:54 Does saturated fat cause insulin resistance? 1:36:30 Do Cyrus and Robby take any supplements? 1:40:54 Leucine and MTOR. 1:44:30 Why Paul went on the carnivore diet/dissenting opinions on nutrition. 1:54:54 The most radical thing Cyrus and Robby have done recently. David A. Sinclair, Ph.D., A.O. is a Professor in the Department of Genetics at Harvard Medical School. He is best known for his work on understanding why we age and how to slow its effects. His research has been primarily focused on the sirtuins, protein-modifying enzymes that respond to changing NAD+ levels and to caloric restriction (CR) with associated interests in chromatin, energy metabolism, mitochondria, learning and memory, neurodegeneration, and cancer. The Sinclair lab was the first one to identify a role for NAD+ biosynthesis in regulation of lifespan and first showed that sirtuins are involved in CR in mammals. They first identified small molecules that activate SIRT1 such as resveratrol and studied how they improve metabolic function using a combination of genetic, enzymological, biophysical and pharmacological approaches. They recently showed that natural and synthetic activators require SIRT1 to mediate the in vivo effects in muscle and identified a structured activation domain. They demonstrated that miscommunication between the mitochondrial and nuclear genomes is a cause of age-related physiological decline and that relocalization of chromatin factors in response to DNA breaks may be a cause of aging. If you are interested in lipids, you’ve probably heard of Dave Feldman and his work. If not,  you’ve got some homework to do, and I think you’ll be amazed at what you find. Dave’s work can found at cholesterolcode.com where he details his multiple, meticulous self-experimental projects. Prior to cholesterol adventures Dave was a senior software scientist and an engineer. He brings this “out of the box” thinking to the world of medicine and we are all better off for it.  He began a low-carb, high-fat diet in April 2015 and has since learned everything he could about it with special emphasis on cholesterol. He saw his own lipid numbers spike substantially after going on the diet and spotted a pattern in the lipid system that’s very similar to distributed objects in networks. He’s since learned quite a bit on the subject both through research and experimentation which has revealed some very powerful data. Where he induced a 73 point drop in my LDL-C and a 1115 point drop in my LDL-P.  Siobhan (she'-VAUN) Huggins is an independent researcher focused on lipidology, the immune system, and metabolic diseases. She spends most of her time at cholesterolcode.com where she works alongside Dave Feldman. She has been keto since August of 2016, and on a predominantly carnivorous diet as of October 2017. She has lost 80 lbs, reversed hypertension, and found a passion for research along the way.    Dave’s contact information:   www.cholesterolcode.com  Twitter: @daveketo Instagram: @davefeldmanketo   Siobhan’s contact:   Twitter: @siobhan_huggins Website: cholesterolcode.com      Ancestral Supplements https://ancestralsupplements.com/   Code SALADINOMD on the shopify site to receive 10% off.   JOOVV: www.joovv.com/paul   INSIDER: carnivoremd.com   My contact information: PATREON: https://www.patreon.com/paulsaladinomd   SOCIAL MEDIA  Instagram: @carnivoremd Website: carnivoremd.com Twitter:@carnivoremd  Facebook: Paul Saladino MD email: paulsaladinomd@gmail.com   Be sure to subscribe for more medicine and lifestyle content. Dr Gary Fettke is an Orthopaedic Surgeon practising in Launceston, Tasmania, Australia. He has a major interest in the preventative medicine and encourages his patients to lose weight before undergoing surgery. Although his speciality is surgery, Gary believes it is much better to help people avoid surgery by taking preventative measures, which often involves primarily altering the diet. He is an advocate of a Low Carbohydrate Healthy Fat (LCHF) lifestyle for the health benefits. He was “a fat kid” and has experienced ‘too many’ other health issues until changing his diet and lifestyle. He has concerns that as we get older the health systems will be under significant stress from financial and health burden aspects. It is simply unsustainable – a train crash in evolution. In recent years Gary has focussed on the role of diet in the development of diabetes, obesity and cancer. He has been speaking out internationally on the combined role of sugar, fructose, refined carbohydrates and polyunsaturated oils linking together to be behind inflammation and modern disease. Gary remains committed to understanding the central role of nutrition in everything ranging from our health issues to its role in influencing education, the environmental impact, economics, politics and belief systems. He has incurred the wrath of regulatory bodies for his stand on public health, but he and his wife, Belinda, remain active defending the benefits of low carb healthy fat living. Peter Ballerstedt earned his bachelor’s and master’s degrees at the University of Georgia and his doctorate at the University of Kentucky.  He was the forage extension specialist at Oregon State University from 1986 to 1992. He currently works in the forage seed industry. Peter’s personal experience has led him to re-examine human diet and health. What he has learned doesn’t agree with the advice given for the past several decades. This new understanding combined with his forage background has given him an increased passion for the key to true social, economic and ecological sustainability – ruminant animal production systems – the source of butter, red meat, and cheese!  Peter has spoken at many different events in the US and internationally. Many of his presentations are available on YouTube (http://bit.ly/2JJDJpE). He blogs at “Grass Based Health” (http://bit.ly/2XaRdxv) and Facebook (http://bit.ly/2wsNBes), and posts to twitter and Instagram @GrassBased. I was getting sick of people throwing shade at the carnivore diet with misrepresentations of the facts so I wanted to fight back with an episode in which I answered each and every one of these criticisms in detail. My buddy Nathan joined me for this one and we dug into it with reckless abandon. The links to the original articles are noted below but SelfHacked.com and Chris Kresser were the folks we were responding to!   https://chriskresser.com/the-carnivore-diet-is-it-really-healthy/ https://selfhacked.com/blog/carnivore-diet/ Nathan’s contact info: @nathanequalsone on twitter Ancestral Supplements https://ancestralsupplements.com/   JOOVV: www.joovv.com/paul   My contact information: PATREON: https://www.patreon.com/paulsaladinomd   SOCIAL MEDIA Instagram: https://www.instagram.com/paulsaladinomd Website: paulsaladinomd.com Twitter:@mdsaladino Facebook: Paul Saladino MD email: paulsaladinomd@gmail.com   Be sure to subscribe for more medicine and lifestyle content. He brings this “out of the box” thinking to the world of medicine and we are all a better off for it.   He began a low-carb, high-fat diet in April 2015 and has since learned everything he could about it with special emphasis on cholesterol. He saw his own lipid numbers spike substantially after going on the diet and spotted a pattern in the lipid system that’s very similar to distributed objects in networks.   He’s since learned quite a bit on the subject both through research and experimentation which has revealed some very powerful data (see his Cholesterol Code series Part I, Part II, Part III, Part IV, and Part V). As of this writing, his “Extreme Drop” experiment has gotten the most attention (see infographic here, and do-it-yourself page here) where he induced a 73 point drop in my LDL-C and a 1115 point drop in my LDL-P.   In this show we discuss Dave’s recent talk at Keto Salt Lake which can be found on YouTube here: https://www.youtube.com/watch?v=UZv00mMiB9M   Dave’s contact information:   Www.cholesterolcode.com Twitter: @daveketo Instagram: @davefeldmanketo     Don’t forget to support Ancestral Supplements https://ancestralsupplements.com/   My contact information: PATREON: https://www.patreon.com/paulsaladinomd   SOCIAL MEDIA Instagram: https://www.instagram.com/paulsaladinomd Website: paulsaladinomd.com Twitter:@mdsaladino Facebook: Paul Saladino MD email: paulsaladinomd@gmail.com   Be sure to subscribe for more medicine and lifestyle content. If you don't know who the Bell brother's are, you've probably been living under a rock! These are some pretty darn incredible dudes that no quick bio can do justice to but here’s my shot. Simply put, Mark Bell is basically super human. He calls himself a “meathead millionaire” but don’t let that fool you. He’s invented numerous powerlifting products, most notably the slingshot, which have changed the way people lift large amounts of weight and made him into an icon in that sport. He also founded the largest free (yes you heard that right) gym in the word, in Sacramento, California, where he continues to train on a daily basis. His past powerlifting accomplishments are jaw dropping but the thing I appreciate most about Mark is what a genuine, good human he is. He tells a bit of his story in this episode, you don’t want to miss it. Chris Bell’s story is equally amazing. Previously addicted to drugs and alcohol, now shredded and lifting huge amounts of weight with two artificial hips whilst eating a 99% carnivorous diet. He’s well know for his much acclaimed film projects including Bigger Stronger,Faster,  Prescription Thugs, and a Leaf of Faith. He’s currently working on a project about human nutrition and the ways we’ve all been led astray. In this episode we talk about all kinds of cool things including the story of Mark’s invention of the slingshot, why he never gave up on this, Chris’ take on the nose to tail carnivore movement, both of the brother’s experiences with the carnivore diet and where they think the future of food is going. Please enjoy this conversation with the Brother’s Bell.
