Although this is a real syndrome which has been recognised for many years and has many serious potential consequences there are many controversies regarding it&#8217;s physiology and treatment. In recent years advances in imaging technology and recent studies have questioned some of practices which were taught as if they were dogma&#8230;&#8230;</p>
