Sometimes we make the same mistake over and over and over again.  With exactly the same disastrous results.  A bit like Pavlov’s dogs.  Stimulus - response.  Stimulus - response.  A vicious circle and it causes pain repeating the same mistakes all the time.  So how do we break out of that cycle??
